• Elder of Famine: also uses the I'm Drowsy... pill effect.
• Elder of Pestilence: also spawns a lingering damaging poisonous ring.
• Elder of Wrath: explosion damage bumped to 100. Grants +0.3 flat damage for the duration of the room.
• Elder of Death: kills non-boss enemies. Deals an additional 9x Isaac's damage to bosses.
• Elder of Conquest: gets a 33% chance to be respawned.
• Elder of Fire: also spawns a Red Candle flame.
• Elder of Light: spawns multiple pillars of lights in a cross pattern.
• Elder of Love: permanently charms non-boss enemies. Deals an additional 5x Isaac's damage to bosses.
• Elder of Thunder: also spawns a Micro Battery.
• Elder of Wisp: also gives Isaac a Lemegeton item wisp.
Unlock: Defeat Delirium as The Rotten.
